29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:       a.)     On or about December 12, 2019, employees were exposed to amputation and crushing injuries when performing change over and maintenance activities on the 2K Tube Mill (Abbey Etna 2KU). The employer did not ensure energy control procedures were documented and utilized to properly control the electrical energy before entering the mixer.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:     a.)     On or about December 12, 2019, the employer did not ensure employees were protected while performing service and maintenance activities on the 2K Tube Mill. Employees were required to perform change over activities on equipment including, but not limited to the 2K (Abbey Etna 2KU) Tube Mill and did not place a lock on the disconnect switch, exposing them to amputation and crushing injuries.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:     a.)     On or about December 12, 2019, the employer did not ensure that employees were adequately trained in the lockout procedures for the 2K Tube Mill.  Employees did not properly lock out and tag out the electrical panel while performing change over activities on equipment including, but not limited to the 2K (Abbey Etna 2KU) Tube Mill.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a.)     On or about December 12, 2019, the employer did not ensure employees were protected while operating the Bridgeport Vertical Mill. Employees were required to operate the drill press for various fabricating activities and are exposed to caught by hazards from the rotating chuck.           b.)     On or about December 12, 2019, the employer did not ensure employees were protected while operating the tube mills, including but not limited to the 2K Tube Mill. Employees were required to monitor the cutoff on the 2K Tube mill, and are exposed to amputation injuries in that it is not adequately guarded.
