29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million particles of air (50 ppm) for any 8-hour work shift of a 40-hour work week and attenuated accordingly for extended work shifts in extended work weeks:  (a) On or about November 29, 2019, an employee was fatally exposed to carbon monoxide levels at a mean calculated 676.3 ppm for approximately 360 minutes, for a 8-hour Time-Weighted average of 507.3 ppm while working in the pool heater rooms at the Baymont Inn & Suites located at 2611 Old Red Trail NW in Mandan, North Dakota.

29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:  (a) On or about November 29, 2019 an employee was fatally exposed to carbon monoxide where continuous monitoring equipment was not provided while working inside the pool heater rooms at the Baymont Inn & Suites located at 2611 Old Red Trail NW In Mandan, North Dakota.
