29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:     On 12/13/2019, employees of Poeling Construction were connecting a 4 inch sewer system to a residential building located at 701 Tinsley Court, Cottleville, Missouri.  The employer did not provide or ensure that a safe means of egress was located in the excavation.  The trench used by employees measured approximately 26 feet in length, 5 feet wide and 5.5 feet deep.

The employer did not ensure a competent person inspected the excavation and surrounding areas to identify any potential hazards prior to the start of work.
