29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:  a)  At the jobsite, on or about 12/13/19:  Employees were working in close proximity to a live service panel that was not guarded with a cover exposing workers to live electrical circuits.  b)  At the jobsite, on or about 12/13/19:  Employees were regularly plugging into live outlets that were not covered exposing those workers to live electrical circuits.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a)  At the jobsite, on or about 12/13/19:  Employees were using a 4 foot stepladder to access a window opening into a building where that ladder was leaned against the building and not opened with the spreaders locked as is the design of this ladder.  This use necessitated stepping onto the top level of the ladder when accessing or egressing the building.
