On a wooden pallet that had been placed on the forks of a powered industrial truck, located at or near 11390 Square Street in Jacksonville, Florida: On or about December 17, 2019, the employer exposed an employee to struck-by and fall hazards, in that a safety platform or cage was not utilized by an employee who stood on the pallet, while performing masonry brick work.

On the forks of a powered industrial truck, located at or near 11390 Square Street in Jacksonville, Florida: On or about December 17, 2019, the employer exposed an employee to struck-by and fall hazards, in that a means was not provided or utilized by an employee to shut off power to the truck as the employee stood on a pallet that had been placed on the truck forks, while performing masonry brick work.

Supervisors' (including foremen) safety and health responsibilities understood. C. Employees adherence to safety rules. 3. Identification and Control of Hazards A.
