29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    On or about December 12, 2020, Employees are exposed to struck-by hazards when operating a CAT forklift with a Cascade lift truck attachment, and the employer did not get the forklift manufacturers written approval before modifying the attachment of the powered industrial truck.
