29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of a Bridgeport Mill (Coded 11040019) without a guard around the rotating chuck.    b) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of a Buffalo gang drill (SN 56-8493) without a guard around each rotating chuck.     c) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of an Allen gang drill (SN 678-12-27) without a guard around each rotating chuck.   d) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of a Bridgeport Mill (SN 95532-2) without a guard around the rotating chuck.  e) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of a Bridgeport Mill (SN J-111873) without a guard around the rotating chuck.    f) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of a Mybea ironworker (SN 01553 84 827 311) without sufficient guarding.     g) At the CNC Mill area, on or about 12/18/2019: The employer permitted operation of an unguarded HydMech saw.   SPECIALTY WELDING & FABRICATING OF NY INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1910.212(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1037336, CITATION NUMBER 2, ITEM NUMBER 1, ISSUED ON 3/16/2015, AND AFFIRMED AS A FINAL ORDER ON 8/31/2015 WITH RESPECT TO A WORKPLACE LOCATED AT 1025 HIAWATHA BOULEVARD SYRACUSE NY 13208.
