29 CFR 1910.133(a)(1) as required by 1960.8(b): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about December 16, 2019, the employer did not ensure that employees required to spray liquid chemicals such as, but not limited to dipropylene glycol monomethyl ether, sodium gluconate, alkyl dimethyl benzyl ammonium chloride and alkyl dimethyl ethylbenzyl ammonium chloride which are included in smart heavy duty degreaser and Lynx multi-purpose sanitizer when performing cleaning tasks wore protective eye protection. Employees were exposed to eye contact hazards that could result in health effects such as, but not limited to severe eye irritation or eye damage.

29 CFR 1910.132(d)(1) as required by 1960.8(b): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment: On or about December 16, 2019, the employer did not assess the workplace for hazards that would require the use of personal protective equipment. Employees are required to spray liquid chemicals such as, but not limited to dipropylene glycol monomethyl ether, sodium gluconate, alkyl dimethyl benzyl ammonium chloride and alkyl dimethyl ethylbenzyl ammonium chloride which are included in smart heavy duty degreaser and Lynx multi-purpose sanitizer when performing cleaning and sanitizing tasks and were not provided the proper eye protection. Employees were exposed to eye contact hazards that could result in health effects such as, but not limited to severe eye irritation, burns or damage.
