29 CFR 1926.1153(g)(1)(ii): The exposure control plan did not contain a description of the engineering controls, work practices, and respiratory protection used to limit employee exposure to respirable crystalline silica for each task:  a)  150 Bay St., Jersey City, NJ  The employer's exposure control plan did not describe engineering controls and work practices for employees using a jack hammers to remove concrete flooring inside a 20' by 20' enclosure.  On 12/20/2019, employees working inside enclosure during jack hammering were exposed to crystalline respirable silica at an 8-hour time-weighted average (TWA) of 73.1 micrograms per cubic meter (ug/m3),  89.1 micrograms per cubic meter (ug/m3), and 77.6 ug/m3, which are 1.5, 1.8 and 1.5 times the permissible exposure limit (PEL) of 50 ug/m3. Zero exposure was assumed for unsampled periods of 210 minutes, 195 minutes and 214 minutes respectively.  Condition occurred on or about 12/20/2019
