29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.     On or about December 19, 2019, at the jobsite, the employer did not ensure employees working below the areas where roofing work was being conducted were in use of a protective helmet.  This exposed the employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.      On or about December 19, 2019, at the jobsite, the employer did not ensure that employees were in use of safety glasses while using hammers and pneumatic nail guns to install roofing materials to the roof of a building.   This exposed the employees to eye hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     On or about December 19, 2019, at the jobsite, the employer did not ensure that employees, observed installing roofing materials to a steep roof, were in use of fall protection.  This exposed the employees to a fall hazard greater than six(6) feet to the concrete below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.     On or about December 19, 2019, at the jobsite, the employer did not ensure that the side rails of a ladder extended at least three (3) feet above the landing on a steep roof.  This exposed the employees to a fall hazard.
