29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about December 12th, 2019, and at times prior thereto, employees were performing roofing activities at a residential construction site which had very large piles of rough stones and debris at the front of the site., exposing employees to the hazard of tripping or falling onto the rocks.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about December 12th, 2019, and at times prior thereto, employees installing shingles while conducting residential roofing operations at heights greater than 6 feet on surfaces with unprotected edges were not required to wear fall protection, exposing employees to the hazard of falling from height.
