29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Old Trail Road, Selinsgrove, PA: Employees receiving hoisted material on the roof of a structure approximately 40 feet above the ground were not protected from falling, on or about December 20, 2019.  b) Old Trail Road, Selinsgrove, PA: Employees installing guardrails along the edge of the roof on a structure that was approximately 40 feet above the ground were not protected from falling, on or about December 20, 2019.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the opening:  a) Old Trail Road, Selinsgrove, PA: A hole in the roof that was used to provided ladder access was not equipped with guardrails or other adequate means to protected employees from a fall, on or about December 20, 2019.  ABATEMENT CERTIFICATION REQUIRED
