29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about December 21, 2019, an employee working on the ground below active roofing operations was exposed to overhead struck-by hazards.  The employee was not wearing head protection.      Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 21, 2019 employees using a ladder to access/egress a roof while engaged in roofing operations were exposed to fall hazards.  The extension ladder being used for access to the roof did not extend three feet above the roof eave/landing surface.     Abatement certification is required for this item in accordance with 29 CFR 1903(c).
