On or about December 26, 2019, in the thread shop where employees were exposed to slipping hazards due to leaking hydraulic fluid covering the floor behind the NOV bucking machine on line #1.    b.  On or about December 26, 2019 and at times prior thereto, in the thread shop where employees were exposed to slipping hazards due to leaking hydraulic fluid covering the floor behind the NOV bucking machine on line #2.

On or about December 26, 2019, in the bucking unit area of the thread shop where employees were exposed to caught-in and struck-by hazards while performing maintenance and servicing work on a bucking unit on line #1 without utilizing energy control procedures.    b.  On or about December 26, 2019, and at times prior thereto, in the bucking unit area of the thread shop where employees were exposed to caught-in and struck-by hazards while performing maintenance and servicing work on a bucking unit on line #2 without utilizing energy control procedures.    Pinnacle Machine Works, LLC was previously cited for a violation of this Occupational Safety and Health Standard  1910.147(c)(4)(i), which was contained in OSHA inspection number 1253288, citation number 1, item number 1 and was affirmed as a final order on September 21, 2017 with respect to a workplace located at 10521 Sheldon Road, Houston, TX 77044.

On or about December 27, 2019, near line#1 in the thread shop where the pipe roller table disconnect switch by the blowout station was not labeled nor arranged so the purpose was evident.
