29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a)  On or about January 1, 2020, an employee painting the awning of the exterior of a commercial building, while standing on the work platform of a JLB Boom Lift Model 400S, was exposed to a 10-foot fall hazard, in that a body belt with lanyard attached to the boom or basket was not used.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a) On or about January 1, 2020, an employee painting the awning of the exterior of a commercial building, while standing on the work platform of a JLB Boom Lift Model 400S, had not been trained to recognize overhead crushing hazards, such as, not facing the direction of movement towards a lower overhang.
