29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  (a) On or about January 2, 2020, the ladder jack scaffold used for installing siding, was erected less than three (3) feet from the 220 volt electrical power line drop to the structure. This exposed employee(s) working on the scaffold to an electrical shock hazard.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      (a) On or about January 02,2020, the portable Werner extension ladder erected on the East side of the Structure, and being used for access to the flat roof, did not extend at least three (3) feet above the gabled roof edge, and/or was not secured to the structure when it was not sufficiently extended.
