29 CFR 1926.453(b)(2)(v):   A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  This violation was observed on or about December 11, 2020 at 14307 NE 23rd St, Choctaw, Oklahoma, the employer did not ensure each employee was properly wearing a safety harness with lanyard attached to the proper anchor points while in the JLG aerial lift, exposing employees to a fall hazard to the ground below.
