Employees manufacturing dyes are exposed to the hazard of permit-required confined spaces. The mixers, and tanks do not have signs indicating that they are permit required confined spaces. The condition was noted at or about 01/03/2020           NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

Employees providing maintenance to the mixers and tanks used in the manufacturing plant are exposed to the hazard of permit-required confined spaces. The employer did not provide training to the entrant employee. The condition was noted at or about 01/03/2020     (b) Jos H Lowenstein & Sons Inc. 420 Morgan Ave. In the dry, and wet process. Employees providing maintenance to the mixers and tanks used in the manufacturing plant are exposed to the hazard of permit-required confined spaces. The employer did not provide training to the attendant employee. Employees providing maintenance to the mixers and tanks used in the manufacturing plant are exposed to the hazard of permit-required confined spaces. Employees were assigned rescue duties where an entry into a pipe steam pipe was conducted. The employer did not ensure that the rescuers had made a practice entry into the steam pipe in the last 12 months. The condition was noted at or about 01/03/2020.
