Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about December 31, 2019, employees were performing roof operations at heights over 6 feet above a lower level and were not protected from falling, exposing employees to the hazards of falling from heights 6 feet or more to a lower level.
