29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about December 31, 2019, employees were performing roof operations and were not protected from falling through roof panel skylights that were more than 6 feet above a lower level, exposing employees to the hazard of falling from heights greater than 6 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about December 31, 2019, employees were performing roof operations at heights over 6 feet above a lower level and were not protected from falling, exposing employees to the hazards of falling from heights 6 feet or more to a lower level.
