29 CFR 1926.304(g)(1): The sides of the lower blade were not guarded to the full diameter of the blade by a device that would automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection.  a) On or about January 8, 2020, on the north side of the jobsite, employees utilized a Makita model 2414NB Miter Saw that did not have the lower blade guard attached to cut pvc pipe for plumbing.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) On or about January 8, 2020 on the north side of the jobsite, employees utilized an extension cord that had a missing ground prong. The cord was used to power a miter saw used to cut pvc pipe for a plumbing installation.
