29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about November 27th, 2019, and at times prior thereto, the employer did not provide a training program for employees using ladders to paint the exterior of residential homes, exposing employees to the hazard of falling from a ladder.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On November 27th, 2019, at approximately 2:00 PM, an employee suffered a fall that resulted in an injury requiring inpatient hospitalization. The hospitalization was reported to the Austin Area Office on December 5th.
