29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 9, 2020, at the site, the worker was standing underneath work being performed above his head. The employee was not wearing head protection. The employee was exposed to a struck-by hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about January 9, 2020, at the site, an employee was standing on the top rail of a JLG scissor lift. The employee was repairing the metal framing. The JLG scissor lift was a model 3246ES. The employee was exposed to a fall hazard of approximately 13 feet to the ground below.
