29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually:    a) Facility-wide: Employees had exposure to blood and bloodborne pathogens during normal work activities and the Bloodborne Pathogen Exposure Control was not reviewed on an annual basis, on or about January 8, 2020.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1030(f)(2)(iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A:  a) Facility-wide: Employees had exposure to blood and bloodborne pathogens during normal work activities and the employer did not require employees to sign a declination form if the employee declined the hepatitis B vaccine, on or about January 8, 2020.   No abatement certification or documentation required.
