29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Maintenance - Employees performing repairs and servicing on mechanical and electrical systems are exposed to hazards associated with the accidental release of stored energy types such as electrical and mechanical (belts, pulleys, fan blades).  Although a process is followed, procedures are not in writing and do not encompass the requirements of this section.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   Maintenance - Authorized employees that maintain electrical and mechanical systems are exposed to the unexpected release of stored energy types such as electrical and mechanical (belts, pulleys, fan blades).  An annual review of the program was not performed by the employer as required.   See required elements (A)-(D)(ii) of this section.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:         Maintenance Shop - A Dayton 8" bench grinder had a 1/2" gap between the grinding wheel and the tongue guard on the left side.  Employees are exposed to impalement hazards.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:   Maintenance Shop - An acetylene cylinder not in use was missing a valve cap.  Employees are potentially exposed to hazards associated with damaged valves.
