29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:   On or about January 14, 2020 during residential roofing activities, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.  John's Roofing Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1320958, citation number 01, item number 01 and was affirmed as a final order on July 31, 2018, with respect to a workplace located at 249 East 327th, Willowick, OH 44095.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 14, 2020, at a worksite located at 635 East Erie Street, Painesville, OH 44077 during residential roofing activities employees were exposed to fall hazards of approximately 19.8 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.  John's Roofing Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1320958, citation number 01, item number 02a and was affirmed as a final order on July 31, 2018, with respect to a workplace located at 249 East 327th, Willowick, OH 44095.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about January 14, 2020, at a worksite located at 635 East Erie Street, Painesville, OH 44077 during residential roofing activities employees were exposed to fall hazards of approximately 16 feet. The employer failed to ensure employees who were exposed to fall hazards were trained in recognizing fall hazards or the procedures to minimize the hazards.  John's Roofing Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1320958, citation number 01, item number 02b and was affirmed as a final order on July 31, 2018, with respect to a workplace located at 249 East 327th, Willowick, OH 44095.
