29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        The employer is failing to ensure that employees are protected from machine hazards. Employees are exposed to the hazard of a mechanical rotating part.  This violation was most recently documented at a worksite located at or near 2500 E 23rd St., Columbus, Nebraska 68601 on or about January 14, 2020. The Wilton drill press was being used by an employee without a guard.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:                                                                                                                                 The employer is failing ensure that employees are protected from struck-by hazards. This was most recently documented at 2500 E 23rd St., Columbus, Nebraska 68601 on or about January 14, 2020. The employer is failing to ensure that the opening between the work rest and abrasive wheel did not exceed one-eighth inch on the 8-inch DeWalt bench grinder.  The opening was one-fourth of an inch.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:                                                                                                        The employer is failing ensure that employees are protected from struck-by hazards. This was most recently documented at 2500 E 23rd St., Columbus, Nebraska 68601 on or about January 14, 2020. Employees were using an 8-inch  DeWalt bench grinder which had the abrasive wheel periphery opening exceeding one-fourth of an inch.
