29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  (a)  On or about January 10, 2020, the employer did not ensure that an employee operating a chainsaw and involved in tree cutting/trimming operations was wearing a protective helmet when working in areas on the ground, where there was a potential for injury to the head from falling objects (tree limbs) above.
