29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    Location: 7 Fosters Point, Beverly, MA    On or about January 14, 2020, a competent person did not perform a frequent and regular inspection of the jobsite to identify safety and health hazards to protect workers from the following hazards;  1. Electric shock hazards, while workers attempted to setup the 36-foot long pole of a pumpjack scaffold within 10-feet of the live 13.2kv power lines and came in contact with them;  2 thru 4. Fall hazards of greater than 21-feet, while workers utilized a pumpjack scaffold without fall protection, no bracing at the bottom of the scaffold, and bracing at the top inadequately secured to the fascia board.
