29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.    On or about 15 January 2020, at the jobsite located at 2 N. Broadway St, Aurora IL. 60506, the employer failed to take the necessary precautions to protect the workers from a shock hazard. Flexible cords were run through floor openings from different levels of the building without being properly protected from damage. This exposed employees to a shock hazard.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c)

29 CFR 1926.852(e): Any chute opening, into which workmen dump debris, shall be protected by a substantial guardrail approximately 42 inches above the floor or other surface on which the men stand to dump the material. Any space between the chute and the edge of openings in the floors through which it passes shall be solidly covered over.    On or about 15 January 2020, at the jobsite located at 2 N. Broadway St, Aurora IL. 60506, an employee who was dumping material at a wall opening on the Third floor were not protected from falls by the use of a guardrail system, a safety net system, or personal fall arrest system. Employees were exposed to the fall hazard when they were dumping material out of the third floor window.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c)
