29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: a) On or about January 15, 2020, and at times prior thereto, employees were exposed to a struck-by hazard, while handling metal decking with sharp edges without being provided gloves. b) On or about January 15, 2020, and at times prior thereto, employees were exposed to a struck-by hazard, while removing old metal decking without safety glasses to protect eyes from flying debris.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about January 15, 2020, and at times prior thereto, employees with no fall protection system in place were exposed to fall hazards, greater than 15 feet to the ground level below while removing and installing metal decking from a roof.

29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. On or about January 15, 2020, and at times prior thereto, employees with no fall protection were exposed to a fall hazard, greater than 15 feet to the ground level below while working from the roof without being trained on fall hazard recognition.
