29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch.    a) Blakeman Vacuum and Sewing, LLC, at 530 North Garfield Ave, Loveland, CO 80537:  On or before January 16, 2020, the employer did not protect employees from injury in that the Reznor, Natural Gas Heater, had openings larger than one half inch on the fan blade on the backside and measured 5 feet, 20 inches from the floor level. The fan was not equipped with a guard/panel on the backside of the unit.  This condition exposed employees to amputation and struck-by hazards.

The employer used temporary power strips and extension cords in the warehouse to power computers, monitors, portable space heaters and other appliances.  This condition exposed employees to fire and electrical hazards.     (b) Blakeman Vacuum and Sewing at 530 N. Garfield Ave, Loveland, CO 80537: On or before January 16, 2020, the employer used portable power electrical strips, daisy chained together with other portable electrical power strips in the main building and was not installed in accordance with its listing.
