29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:    (a)Black Hills Industries. Inc. dba Black Hills Roofing at 14 Wolf Creek Road, Pine Ridge, SD 57770: On or about and at times prior to January 14, 2020, the exposing employer did not ensure that the warning line system being used for fall protection was erected around the entire work area. Employees were working on the roof of a new commercial roofing project with inadequate warning lines that did not encompass the work area. This condition exposed employees to a fall hazard of approximately 16 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:       (a)Black Hills Industries, Inc. dba Black Hills Roofing @ 14 Wolf Creek Road, Pine Ridge, SD 57770: On or about and at times prior to February 14, 2020, the exposing employer did not ensure that each employee engaged in roofing operations on a commerical building, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The employer did not provide training on the warning line system involving it's limitations and use. The condition exposed employees to the hazard of falling from an elevation of approximately 16 feet.

29 CFR 1926.150(c)(1)(iv): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of where ever more than 5 gallons of flammable or combustable liquids or 5 pounds of flammable gas are being used on the jobsite:    (a)Black Hills Industries, Inc. dba Black Hills Roofing at 14 Wolf Creek Road, Pine Ridge, SD 57770: On or about and at times prior to January 14, 2020, the exposing employer did not ensure that employees were protected from fire hazards associated with use of Sika Seam Cleaner (Acetone) which is highly flammable. Employees were using the flammable product to clean the seams of the PVC roof being installed on the Ogalala Lakota County CTE High School. The employer did not ensure that a fire extinguisher, rated not less than 10B, was provided within 50 feet of the flammable liquid being used on the jobsite.
