On or about 15 January 2020, a can of Dura Technologies Inc.  Clear High Gloss Additive was sitting open without the lid in the shop area at the work bench exposing workers to a hazard of fire and/or flash fire.    b)  Maintenance/Repair shop, SW Wall, 1200 Port Authority Rd, Eddyville, KY.  On or about 15 January 2020, a can of Auto Body Master  Wax and Grease Remover was sitting in the open without the cap in the shop area at the work bench exposing workers to a hazard of fire and/or flash fire.

One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    a) Maintenance/Repair shop, SW Wall, 1200 Port Authority Rd, Eddyville, KY.  On or about 15 January 2020, a handmade "box" fan was not guarded to protected workers from contacting the fan blades or point of operation on the motor driving fan blades.

Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a) Maintenance/Repair shop, SW Wall, 1200 Port Authority Rd, Eddyville, KY.  On or about 15 January 2020, a yellow flexible extension cord hanging from a rack in the Southeast Corner of the shop area was improperly spliced exposing workers to a hazard of electric shock.       b) Maintenance/Repair shop, SW Wall, 1200 Port Authority Rd, Eddyville, KY.  On or about 15 January 2020, a orange flexible extension cord hanging from a rack in the Southeast Corner of the shop area was improperly spliced exposing workers to a hazard of electric shock.

On or about 15 January 2020, the outer insulation of the cord was pulled free from the plug of an extension cord used to power an air compressor exposing workers to electrical shock hazards.      b) Maintenance/Repair shop, SW Wall, 1200 Port Authority Rd, Eddyville, KY.  On or about 15 January 2020, the outer insulation of the cord was pulled free from the plug of an extension cord used to power a floor fan exposing workers to electrical shock hazards.
