Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  Location: shop area On or about 1/17/2020, the employer did not obtain manufacturers' approval prior to using 5 foot 6 inch extended forks and a boom attachment on a Toyota model 8FGCU32 fork lift.
