At the jobsite, employees were not wearing hardhats while working below other employees that were framing a residential house, exposing employees to struck-by hazards from falling objects.

At the jobsite, employees operating a Hitachi nail gun to perform framing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.

At the jobsite, employees used a yellow extension cord spliced to an orange extension cord and wrapped with black tape, supplying power to a Makita Skill saw,  exposing employees to the hazard of electrical burns and shock.

Employees were exposed to an unprotected fall of over 9 feet to the ground below.

The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
