On or about January 21, 2020, an employee was exposed to a 12' 6" fall hazard when using an extension ladder to gain access to a commercial roof and the side rails were not extended at least 3 feet above the upper landing surface.

(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail system; Safety net system; or Personal fall protection system, such as personal fall arrest, travel restraint, or positioning systems.     a.)  On or about January 21, 2020, located on the east side of a commercial exterior structure, an employee was exposed to a 12' 6" fall hazard while performing plumbing work on an unprotected edge of a flat roof.
