29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    On or about January 16, 2020, the employer required the employees to operator the Panter King pedestal grinder- LF137 with the left side tongue guard measured approximately one inch away from the abrasive wheel which prevent or eliminate a struck by hazard.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    On or about January 16, 2020, the employer did not have a strain relief for the junction box on the end of the flexible cord in the shipping department which had a compressor attached exposing employees to electrical shock hazard.
