29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a- Entrance Roof: An employee engaged in roofing activities on a 7 on 12 pitched entrance roof was exposed to fall hazards of approximately 17ft to the concrete below as a ladder side rails used for access did not extend 3ft above the upper landing surface.  Condition noted on or about 01/22/2020.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a- Building Entrance: An employee engaged in roofing activities on a 6 on 12 pitched roof was exposed to fall hazards of approximately 17ft to the concrete below as a means of fall protection was not utilized. Condition noted on or about 01/22/2020.
