29 CFR 1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used.    a) On 1/14/2020 an employee was in the process of moving a press mold (approx. 3400 lbs.) utilizing a 2 ton overhead crane.  The employee attached the press mold to the cranes hook using a double wire rope slings (rated lifting capacity 4500 lbs.)The legs of the wire rope was looped over two makeshift 3 ½ X ½ hex bolts (not rated for lifting), the bolts were threaded into the mold.  As the employee began to lift the mold, the employees hand was on the mold to guide it over to destination.  As the slings tightened the employees left hand was caught between the wire sling and the mold.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    a) On or about 1/22/2019 the employer did not properly reduce the PSI for the compressed air utilized by employees for the cleaning of themselves or equipment exposing employees to hazards associated with compressed air measured at greater than 30 PS1.    Test locations as follows:  Test location #1 was at the Press #2, it measured at 100 PSI.  Test location #2 was at a work station, it measured at 50 PSI.
