29 CFR 1910.178(m)(3): A safe place to ride was not provided where riding a powered industrial truck(s) was authorized: On or about January 24, 2020, management officials were exposed to fall hazards on while standing on the forks of a sit down powered industrial truck (Toyota Forklift). The forks were used to elevate the officials 4 feet or more in order to retrieve product. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about January 24, 2020, employees were exposed to struck-by hazards due to training not being provided to management officials prior to the operation of a powered industrial truck. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about, January 24, 2020, employees and management officials were exposed to chemical hazards while being required to change out propane tank(s) mounted on a sit-down Toyota powered industrial truck. Employees and management officials were not provided effective information and training on the hazards to which they were exposed. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
