29 CFR 1926.1053(b)(13): The employer did not ensure that the top or top step of a stepladder was not used as a step:   a) Job site, at or near 936 Sheraton Drive, Mars, PA: On or about January 15, 2020, the employer did not ensure that a stepladder was used safely in that an employee had 1 foot on a rung and was using one of his knees to kneel on the very top section of the ladder.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use:   a) Job site, at or near 936 Sheraton Drive, Mars, PA: On or about January 15, 2020, the employer did not ensure that stepladders were inspected by a competent person for visible defects. Deficiencies noted during the inspection included the lack of proper ladder labeling in that many of the warning/instruction labels were either marred/illegible or were missing.
