29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.   a) Jim Slabaugh Carpentry, Inc, at a worksite located at 1700 Burgess Avenue, Cambridge, Ohio 43725: On and/or preceding January 28, 2020, the employer did not initiate and maintain a safety and health program, such as a fall protection program, for employees conducting framing and roofing work.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) Jim Slabaugh Carpentry, Inc, at a worksite located at 1700 Burgess Avenue, Cambridge, Ohio 43725: On and/or preceding January 28, 2020, the employer did not ensure that employees engaged in commercial construction activities 6 feet or more above lower level were protected from falls. Framers were installing and blocking/bracing trusses while standing at or near the edge of the existing flat roof, 11 feet 1 inch above the ground below. The employees were not utilizing any form of fall protection, which exposed them to falls up to 11 feet 1 inch.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   a) Jim Slabaugh Carpentry, Inc, at a worksite located at 1700 Burgess Avenue, Cambridge, Ohio 43725: On and/or preceding January 28, 2020, the employer did not ensure that employees engaged in roofing activities 6 feet or more above lower level were protected from falls. An employee was installing roofing paper to the roof, 11 feet 1 inch above the ground below. The employee were not utilizing any form of fall protection, which exposed him to falls up to 11 feet 1 inch.
