29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:      a) On top of a 6-story exterior wall of a new construction commercial building, located at, or near, 4905 San Pablo Road in Jacksonville, Florida: On or about January 28, 2020, The employer exposed subcontracted employees to 56-foot fall hazards, in that fall protection was not properly utilized by employees while removing concrete forms.
