On the north side of the Warehouse at or near Dock 14, where employees were exposed to a musculoskeletal hazard when manually handling a dock leveler without the dock leveler maintained in a safe condition.  b. On the north side of the Warehouse at or near Dock 42, where employees were exposed to struck-by and trip hazards when using a dock leveler without the dock leveler maintained in a safe condition.
