On or before January 30, 2020, the employer did not ensure that each employee at the workplace, was instructed in the purpose and use of the energy control procedure and about the prohibition relating to attempts to restart or reenergize machines or equipment which were locked/tagged out.   The Central Aluminum Company, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i)(B), which was contained in OSHA inspection number 1390513, citation number 2, item number 2 and was affirmed as a final order on May 29, 2019, with respect to a workplace located at 2045 Broehm Road, Obetz, Ohio 43207.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a. On or about January 30, 2020, in the extrusion department, the light switch for the light fixture above the operator's work area for the 1675 Wean United Youngstown Foundry & Machine aluminum extrusion press had an opening on the bottom side due to a knockout that was removed, thereby exposing employees to an electrical hazard.   The Central Aluminum Company, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1390513, citation number 1, item number 4 and was affirmed as a final order on May 29, 2019, with respect to a workplace located at 2045 Broehm Road, Obetz, Ohio 43207.
