29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Jobsite: The employer did not ensure that each employee who was exposed to fall hazards while re-roofing a flat roof was protected from fall hazard by the use of appropriate fall protection systems/devices or other safe means.      Windsor Home Improvement LLC was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1926.501(b)(10) which was contained in OSHA Inspection Number 1402626, Citation Number 1, Item Number 2 and was affirmed as a final order on July 9, 2019, with respect to a workplace located at 39 Sumner Street, Hartford, CT 06101.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    Jobsite:  The employer did not verify through a written certification that the fall hazard training for the employees had been performed as complete.
