29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At the jobsite, on the main roof, on or about 1/29/20:  Employees were installing roofing materials on an 8:12 pitch roof, with a ground to eave height of 20 feet, without fall protection when working outside guardrails.  b)  At the jobsite, on the rear roof, on or about 1/29/20:  Employees were installing roofing materials on an 8:12 pitch roof, with a ground to eave height of 20 feet, without fall protection.    c) At the jobsite, on the rear roof, on or about 1/29/20:  Employees were installing roofing materials on an 8:12 pitch roof, with a ground to eave height of 20 feet, without properly installed anchorage.    d) At the jobsite, on the rear roof, on or about 1/29/20:  Employees were installing roofing materials on an 8:12 pitch roof, with a ground to eave height of 20 feet, without providing for less than 6 foot swing fall.  e) At the jobsite, on the rear roof, on or about 1/29/20:  Employees were installing roofing materials on an 8:12 pitch roof, with a ground to eave height of 20 feet, with more than one person attached to a anchor.       Proctor Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(11), which was contained in OSHA inspection number 1119355, citation number 1, item number 1, and was affirmed as a final order on 3/10/16, with respect to a workplace located at Lafayette Street, Geneva, NY.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At the jobsite, accessing the rear roof , on or about 1/29/20:  Employees were accessing a roof, 20 feet from ground, with an extension ladder that did not extend at least 3 feet above the upper landing.    Proctor Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1156445, citation number 1, item number 2,  and was affirmed as a final order on 11/29/16, with respect to a workplace located at 120 Union Street, Newark, NY.
