29 CFR 1910.28(b)(13)(i): When work is performed less than 6 feet (1.6 m) from the roof edge, the employer did not ensure each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. Location: a) On the roof toward the southeast side of the building: On or about 02/04/2020, employees were hanging banners at the edge of the roof and were exposed to falling approximately 20 feet to the lower level. Employee were not protected from falling by the use of by guardrail system, safety net system, travel restraint system, or personal fall arrest system.

29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, was not worn and attached to the boom or basket when working from an aerial lift. Location: a) On the aerial lift basket at the jobsite: On or about 02/04/2020, employees were hanging banners on the aerial lift basket without wearing a personal fall arrest or travel restraint system. The aerial lift was also used by employees to access the roof. The employees were exposed to falling approximately 15 feet to the lower level.
