A worker on the ground level was operating an electrical saw, cutting material, without eye or face protection. The worker was exposed to a struck-by hazard.

Woodlawn Avenue, Kirkwood, MO 63122, on or about 2/3/2020, workers were observed working from a ladder jack scaffold, approximately 14ft above lower level, in front of the house. The ladder jack scaffolding consisted of three ladders and two platforms. The platforms were bridged together on the center ladder. The workers were exposed to a fall hazard.
