29 CFR 1910.28(b)(11)(ii):Duty to have Fall Protection and Falling Object Protection (ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) In the basement stairwell, employees descending the enclosed wooden stairway were exposed to a fall hazard as the stairway with 12 risers leading down to the landing was lacking at least one handrail.

29 CFR 1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. In the basement floor area, an uncapped, orange CO2 gas cylinder was located next to the soda dispensing system and located to where it could be knocked over.
